Case Study 1:
Lonnie, a 3-year-old boy was an only child who lived with his parents. His mother, who was home with Lonnie during the day, was having trouble with Lonnie’s disruptive behavior. He whined frequently, interrupted her when she was busy, and constantly demanded that she play with him. Often, he would end up tantruming on the floor, and throw things if his mom didn’t respond. His mother responded in various ways, sometimes she played with him, sometimes she explained to him that she was busy, and sometimes she ignored the disruptive behaviors. She reported that on most days, Lonnie woke up and had breakfast. After breakfast, Lonnie’s mom did errands with Lonnie, or she completed chores around the house, while Lonnie played. On some days, she took him to the library or for a playdate. Around noon, Lonnie and his mom ate lunch together, and then Lonnie napped until 1:30. After his nap, Lonnie watched TV while his mom prepared dinner, and then his dad came home. Once his dad was home, often Lonnie would play with his Dad until dinnertime. After dinner, Lonnie took a bath and watched TV until bed. Lonnie’s mom identified that the days where she had activities planned for Lonnie were better, but that she could not always plan activities or give him individual attention, since she had things to do around the house or for the family. Lonnie’s mom went to her family pediatrician to get help with the behavior, and he asked her to keep track of (1) What time of the day the behaviors seemed to happen,

How long Lonnie was able to play by himself, (3) What Lonnie’s favorite things were to do alone, and (4) What things Lonnie liked to do with her? In addition, he asked her to identify what seemed to trigger the episodes and what she did in response. The pediatrician stated that understanding the patterns would help them come up with a strategy. The data are discussed below:
Time of the day: Lonnie’s Mom reported that over the course of the week, 17 episodes occurred in the morning, between breakfast and lunch, and 5 episodes

occurred after nap. Lonnie did not engage in any behaviors from the time his Dad came home until dinnertime. After dinner, he had 3 episodes of the week. Lonnie was never reported to engage in the disruptive behaviors during meals.

Play Behavior: Lonnie’s Mom reported that during the week, Lonnie played alone on average for about 10 minutes, though some days it was as short as 5 minutes, and some days he seemed to be able to entertain himself for 30 minutes. She did say he could watch TV alone for up to an hour, but that she did not want him doing that consistently, as she knew it was not the best option for him.
Alone Play Preferences: Lonnie’s mom observed that when alone, Lonnie was most successful with his puzzles, blocks, and trains. While he seemed to like coloring and painting alone, she did not feel comfortable letting him play alone with those because he would paint or color on furniture.
Together Play Preferences: Lonnie’s mom observed that when they played together, Lonnie most frequently requested going outside and playing on the play set, doing arts and crafts, or playing with his dinosaurs.
Triggers to the Episodes: Lonnie’s mom reported that of the 25 episodes that occurred over the week, 15 of them occurred in when Lonnie’s mom was completing chores in another room, Lonnie couldn’t figure out what to play with, and she had told him that he needed to figure it out and play alone. 7 of them occurred when Lonnie’s mom was in another room but had just checked on Lonnie and he was independently playing or watching TV. 3 of the episodes occurred out of the blue, when Lonnie’s mom and dad were talking and Lonnie was watching TV.
Responses to the Episodes: Lonnie’s mom reported she was inconsistent during the week of observing. 10 times she ignored the disruptive behavior but found it unmanageable to do so as he escalated and began to throw items consistently, then she had to respond and bring him along with her to complete chores (which she said was still unsuccessful). 8 times she went and played with him after going back and forth, trying to get him to play alone. 5 times, she immediately went and played with him, because she reported she just couldn’t get anything done otherwise.
