Presenting Problem

Charlotte is seeking treatment for her anxiety and depression that stems from ongoing family stressors, lack of self-esteem, and motivation. She reports experiencing a variety of symptoms related to anxiety and depression over the last 6 months. Charlotte is looking for help in learning how to decrease her symptoms while increasing her confidence and motivation. Charlotte is of the Christian faith and reports being heterosexual. She currently lives where she grew up at least within the same area. She reports that her symptoms she has been experiencing have worsened over the last couple weeks, along with a lack of appetite and an increase sleeping habit. Charlotte self-reports the use of marijuana in which she indicates that she has been using for 12 years. She reports no suicidal or homicidal ideations. Charlotte reports that under the influence of alcohol at a party she was sexually assaulted and that she blames herself partially and has not used this substance since the incident. Charlotte has not disclosed this assault to anyone including her previous therapist. Charlotte is currently at New Changes Outpatient Behavioral Health Treatment Facility. Charlotte has had a stent with inpatient treatment following a breakup from a verbally abusive boyfriend. Charlotte reports that she believes the explosive behavior is something that she has acquired from her father who is an alcoholic and also suffers from anger issues. Her mother also takes medications for depression so there is a history of substance abuse and mental health within her family. |

Charlotte is single with no children and of the Christian faith. Charlotte does not currently hold a job but reports interest in obtaining one in which she is able to maintain employment with, as well as the potential of enrolling in college. Charlotte has been without a job for 3 months and has had a history of not being able to maintain employment anywhere. Charlotte has relied on her parents in the past for help with financial difficulties, but they have since cut her off and do not agree with her choice in not attending school, as well as being unhappy with her inability to maintain employment. Charlotte is currently living with a former coworker at this time but is between living arrangements. Charlotte was in a four-year relationship with the break up resulting in an inpatient hospitalization at the age of 24 due to extreme depressive symptoms that she experienced afterwards. She reports that ex-boyfriend was verbally abusive and called her names such as “lazy” and “stupid”. She admits to destroying property during these times of anger. Once punching holes in the wall after an argument with her parents. Charlottes parents and brother are college graduates with respective jobs in the medical field. She feels pressured from what she feels that her parents want of her and feels as though she cannot measure up to their wants. Charlotte reports that at an early age she began to struggle with school and has a learning disability. She was one of 20 African American students within her school where she felt as though she was treated differently. She comes from an upper middle-class neighborhood. |

Charlotte is a heterosexual African American female of Christian faith who is 28 years old. Charlotte came from an upper middle-class upbringing as her parents were college graduates and hold respective jobs in the medical field. Her brother was also successful in graduating college and is now a veterinarian. Charlotte reports feelings of not being good enough for her parents and their expectations and this has caused pressure and overwhelmingness for her. Charlotte reports being only one of 20 African Americans in her school where she felt as though she was, she was treated differently. She reports a learning disability at an early age in which caused her to struggle academically throughout her time in school. However, she reports that she potentially wants to now enroll in college courses. Charlotte reports that she delf medicates with marijuana and has been since the age of 16. She also reports that at a party she consumed alcohol and was sexually assaulted at a party in high school and has not used since. This traumatic experience has not been discussed with anyone, and Charlotte does not report a history of criminal actions as a result of any of this. |
